Facility Evaluation Report
On 01/22/2026, Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived unannounced at the facility to conduct a case management visit regarding the effective exclusion of an individual. LPA met with staff Emerinciana (Amy) Estrada who then contacted Administrator Sharon Thompson via telephone. LPA explained to Administrator the purpose of the visit.
LPA and Administrator discussed the letter was received on January 21, 2026, and facility has since comply with the exclusion.
Administrator stated that the individual has been separated from the roster.
Exit interview and a copy of the report was provided. Signature was received by staff as Administrator was not at the facility. Signature is acknowledge receipt of the report.
