Facility Evaluation Report
On 03/12/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a required- 1 year annual inspection utilizing the care tool. LPA met with Administrator Amanda Britt and explained the purpose of the visit. During today's visit two (2) residents were present in the facility and one (1) was at day program. Additionally, there were three (3) Direct Support Professional (DSP) staff present at the facility.
LPA and Staff conducted a tour of the facility. Areas toured included but not limited to the kitchen, dining room, resident bedrooms, bathrooms, common areas and backyard. LPA observed sufficient furniture and lighting throughout the facility. LPA observed the facility to have sufficient food supplies for seven (7) day non-perishable and two (2) day perishable. LPA observed centrally stored medications, knives and toxins to be locked and inaccessible to residents in care. The temperature in the facility was 70 degrees Fahrenheit. Fire extinguishers was last inspected on 09/23/2024. Smoke detectors and carbon monoxide detectors are working and present throughout the facility.
LPA reviewed three (3) resident records, which contained all required documents. LPA reviewed three (3) staff files. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current training completed.
LPA completed the full care tool and no deficiencies was observed.
Exit interview conducted and a copy of the report was provided.
