03/03/2026 and conducted by Evaluator Talwinder Bains

COMPLAINT CONTROL NUMBER: 59-AS-20260303131435

Staff did not Provide Care and Supervision.

On 5/20/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to deliver final findings for above allegations. LPA met with Administrator Amanda Britt and explain the purpose of the visit.

The department conducted record review, interviewed residents and staff to investigate this allegation. From the information gathered, it was learned that resident, R2 left facility without staff’s supervision in the afternoon hours and was brought back to the facility uninjured by staff. R2 was receiving 1:1 care and supervision from facility 24/7 based on R2’s IPP. Staff did not provide proper supervision resulting in R2 leaving the facility without supervision therefore, this allegation was Substantiated. Based on the information gathered, the preponderance of evidence standards has been met.Therefore, the above allegation is found to be SUBSTANTIATED.

Citation was issued as listed on LIC9099-D per Title 22 Regulations.

Exit interview was conducted, appeal rights and a copy of this report was provided.
