Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Parks arrived on Wednesday April 8, 2026 to conduct a case management visit in regards to an incident the Department received on April 3, 2026.

LPA met with Administrator Amanda. LPA learned the following: On 4/1/2026, R1 was sleeping in the bathroom (per R1's routine). Staff smelled bleach. Upon investigation, staff found that R1 was pouring bleach on their clothing. Staff immediately confiscated the bleach. Per regulation and facility policy, bleach and all chemicals should be kept locked and inaccessible to clients at all times.

LPA obtained a copy of R1's IPP and physicians report.

Exit interview conducted. A copy of this report and appeal rights was provided to the facility.
