Facility Evaluation Report

On 01/06/2026 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct the annual inspection. Upon arrival LPA was informed by Luisa David, staff member and Mother to Licensee that Licensee Liza David passed on 05/12/2025. Luisa informed LPA that they had sent a letter to Community Care Licensing (CCL) notifying CCL of the passing and that they want to close the facility. Additionally, LPA obtained a copy of Licensees Death certificate

The facility has never had any residents.

LPA informed Luisa David that facility will be closed in the department's system effective today, 01/06/2026.
