The inspector’s account

Interviews with staff, clients, and outside sources revealed that on March 17, 2026, C1 arrived to the facility past 10 PM but was allowed access to the facility. Interviews also revealed that C1 was offered their medication. Review of communication threads between the facility and C1 revealed that C1 was aware that they had permission to access the facility.

Based on interviews and record review, the investigation did not yield a preponderance of evidence to conclude that staff locked client out of the facility and staff did not administer client’s medication as prescribed. Based on the foregoing, the allegation is unsubstantiated. This finding means that although the allegation may have happened or may be valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted with Licensee David DiMauro, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
