The inspector’s account

Licensing Program Analyst (LPA) Andrew Martinez made an unannounced visit to the facility to conduct an investigation for Complaint Control Number 56-AS-20260729114136 .

During the visit, LPA Martinez was made aware that staff did not promptly seek medical attention for Client 1 (C1) after sustaining an injury while at the Adult Residential Facility (ARF). Client did not receive medical attention until after Day Program staff contacted ARF Administrator indicating that C1 needed to be picked up for medical care and to obtain medical clearance in order to be allowed back to Day Program the following day.

Based on information obtained, LPA informed Administrator Stien Bawengan a deficiency will be issued as this poses a potential health, safety, and personal rights risk to clients in care. An exit interview was conducted where this Facility Evaluation Report (LIC 809 and LIC 809-D) was discussed and a copy provided to Administrator Stien Bawengan.
