Licensing Program Analyst (LPA) Michelle Echeverria attempted to conduct an annual inspection on 10/25/2025. LPA met with Licensee Latonia Wilson and was granted entry to the facility.
Upon arrival, LPA observed that the facility is no longer operating and the licensee did not have any residents. Licensee stated that she sent her closure letter to the regional office.
LPA spoke with the Licensee, and informed her that there had been no formal closure process completed. The license is still listed as active under Community Care Licensing. Licensee surrendered her license.
An exit interview was provided to the Licensee, Latonia Wilson.
