Facility Evaluation Report

On 6/24/2026, Licensing Program Analyst (LPA) Mary Rico conducted an unannounced case management visit. LPA Rico met with Assistant Executive Director Theresa Gamez and explained the purpose of the visit. The visit pertained to the complaint control number 56-AS-20260116121012. LPA Rico conducted interviews with staff, residents, toured the facility and reviewed facility documents.

Based on the observations made during today’s visit, no deficiencies cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was provided, and a copy of this report was given to Assistant Executive Director Theresa Gamez.
