On 3/16/2026, Licensing program analyst E. Conchas arrived at the facility unannounced to initiate a Case Management Visit to clear out deficiencies. The investigation consisted of observation and review of the pertinent documents. LPA spoke with caregiver, Kevin Joshua Hurd and discussed the purpose of the visit

Based on the observations made during today’s visit, no deficiencies were cited.
An exit interview was conducted, and this report was discussed and provided to Staff at the conclusion of the visit with appeal rights to Caregiver, Kevin Joshua Hurd.
