Facility Evaluation Report

Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit to the facility to obtain additional information. The LPA met with Lead staff Kenneth Caldwell, informed him of the purpose of the visit and was granted access. Administrator Larisa Madera joined the visit at a later time.

LPA's case management included interviews with management and staff, obtaining pertinent documentation and conducting a tour of the facility for a health and safety check. No health and safety concerns were observed during today's visit. Information obtained from interviews with management and staff revealed staff 1 (S1) identified in complaint received on 03-27-25 was the individual who was terminated on 02-15-25. According to the Administrator, S1 will be dissociated from the facility effective today 07-8-25. LPA requested Licensee to send proof of dissociation via text or email.

Administrator Larisa Madera was advised that possible visits and phone interviews will be conducted. An exit interview was conducted, and a copy of this report was provided to Administrator Larisa Madera.
