Facility Evaluation Report
On 02/10/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced case management visit to follow up on an Unusual Incident/Injury Report (LIC 624) received by Community Care Licensing on December 3, 2025 documenting that on November 26, 2025, Staff 1 (S1) reported that Staff 2 (S2) and a previous client, Client 1 (C1) may be in a romantic relationship. The LIC 624 notes S2 was called to be placed on suspension while the organization gathered more information and resigned effective immediately. The LIC 624 notes due to S2's immediate resignation, they are no longer employed with any of licensee's facilities.
LPA toured the facility and did not observe any clients in care or evidence to suggest the home is currently occupied. CS Sumioka reported that the facility has not received client placement since September 25, 2025. CS Sumioka reported that an internal investigation revealed there was no indication S2 had initiated a romantic/personal relationship with any of the clients including C1 while the clients received services in the facility. CS added there are no reports of any additional victims besides C1.
Regional Licensing and Compliance Manager, Crystal Butler reported the facility's Code of Conduct prohibits staff from initiating or maintaining personal contact with discharged clients for two years following discharge, and the facility maintains a zero-tolerance policy prohibiting any sexual contact with clients as well any communication of a flirtatious nature or involving sexual innuendo. LPA requested copies of pertinent documentation to be emailed to LPA by close of business on Friday, 02/13/2026. An exit interview was conducted an a copy of this report was reviewed and provided to CS Sumioka.
