Facility Evaluation Report

On 02/10/2026, Licensing Program Analyst (LPA) Janette Romero made an announced visit for the purpose of the facility's closure. LPA met with Compliance Specialist (CS) Maggie Sumioka. On 12/10/2025, LPA received an email from Regional Licensing And Compliance Manager, Crystal Butler reporting there is an intent to sell the home as there are no clients in care. CS Sumioka reported the facility has not received client placement since September 25, 2025.

During today's visit, LPA inspected the home and confirmed there are no clients in care or evidence to suggest that the home is currently occupied. CS Sumioka also provided LPA with the facility's license. An exit interview was conducted and a copy of this report was reviewed and provided to CS Sumioka.
