Facility Evaluation Report

On 07/10/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. The facility has an approved fire clearance effective 06/07/23, and is a licensed Long term residential treatment program, that can serve six (6) ambulatory clients. During today's visit LPA met with Program Administrator Kenyota Dokes, whom accompanied LPA throughout today's visit.

The facility was observed to be clean, and clutter free with the passageways being free from any obstructions. The facility was observed to have the required postings in the entryway of the facility. The medications, chemicals, and other hazardous items were observed to be locked and inaccessible to clients in care. The facility fire extinguishers and alarm was last serviced on 10/18/24. There are no guns, ammunition or pools or bodies of water on the premises. The facility food supply was observed to be adequate as there is a 2 day supply of perishable and a 7 day supply of non perishable food items. The facility is conducting emergency disaster drills on a monthly basis, the last drill was conducted on 06/30/25.

A records review was conducted, the facility annual fees have been paid, and the governing body is in good standing. Staff files reviewed were observed to have criminal record clearance and to be associated to the facility. Staff were observe to have completed CPR/First aid certification. The client files were observed to have admissions agreements, and assessments. In addition LPA verified facility contact information. There were no deficiencies issued during today's visit.

An exit interview was conducted and a copy of this report was reviewed and provided to Kenyota Dokes, Program Administrator.
