The inspector’s account

On 09/08/2025, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced case management visit to follow up on Report of Suspected Dependent Adult/Elder Abuse (SOC 341) dated 04/24/2025 submitted by the facility involving Client 1 (C1) and Staff 1 (S1). LPA met with Program Administrator (PA), Molissia Butler who was informed of the purpose of the visit.

The SOC 341 notes on an unknown date and time, S1 was observed grabbing and pulling C1 to have them walk with their group and Staff 2 (S2) intervened and told S1 to release C1, which they did. LPA toured the facility with PA Butler, conducted interviews, and obtained copies of pertinent records. Administrator, Ann Marie Stanton reported S1 was immediately placed on administrative leave and has not returned to work. During today's visit, no imminent health or safety concerns were observed and no deficiencies were cited. LPA was informed the facility's management was in a meeting at the time of the exit interview. Therefore, an exit interview was conducted with Staff, Hans Laske and a copy of this report and Confidential Names list (LIC 811) was reviewed and provided to Staff Laske.
