The inspector’s account

On Licensing Program Analyst (LPA), Javina George, made an unannounced visit to the facility to conduct a 1 year required visit. LPA met with Co-Administrator, Krisly Badilla; who was notified of the purpose for the visit. The facility is licensed to serve (4) ambulatory age range 18 through 59.

The facility was observed to be clean and clutter free. The outdoor and indoor passageways are kept free of obstruction. The facility was observed to have the required staple of 2 day supply of perishable and a 7 day supply of nonperishable food items. The appliances were observed to be operable and in good repair. The smoke and carbon monoxide detectors were tested and found to be operable. There are no pool or body of water, or guns and ammunition on the premises. Emergency drills are being conducted on a monthly basis, with the least drill being conducted on 06/01/25.. The hot water was tested and measured outside the required range at 143.2 degrees Fahrenheit, the hot water heater was adjusted. In addition a caution hot water sign was placed in the bathroom.

A records review was conducted the facility annual fees were observed to have been paid. All staff present at the facility were observed to have criminal record clearance and to be associated to the facility. Client files were observed to have completed admissions agreements, Individual Program Plans (IPP) and Medical Assessments. The personal and Incidental funds were reviewed with no discrepancies found.

Staff were observed to have valid First Aid and CPR certification. The administrator, Jessie Castro, has an active Administrator's certificate, that expires on 05/15/26. There were no deficiencies cited during today's inspection. An exit interview was conducted and a copy of this report was reviewed and provided to Krisly Badilla.
