The inspector’s account

On 05/14/2026, Licensing Program Analysts (LPAs) Janette Romero and Jarred Torres made an unannounced case management visit to the facility to address a deficiency observed at the facility. LPAs were greeted and granted entry by Caregiver Alejandro "June" Narrito Jr., who was informed of the purpose of the visit. Administrator Tyla Kincherlow was contacted and also informed of the purpose of the visit. She arrived to the facility to meet with LPAs.

LPAs toured the facility with Caregiver Narrito Jr., conducted interviews, and reviewed records. During the tour, LPAs observed that a bed was placed back into the walk-in closet of the bedroom shared by Resident 1 (R1) and Resident 2 (R2) despite being cited for this on 04/13/2026. LPAs also observed an individual lying on the mattress inside R1 and R2's walk-in closet. This individual identified themselves as Caregiver Narrito's spouse and reported to be visiting Caregiver Narrito since the night of 05/13/2026 and will leave on the evening of 05/14/2026. The individual denied providing any care or supervision to the residents but reported sleeping in the bed inside this walk-in closet. As a result, a deficiency will be cited and a civil penalty will be assessed.

No immediate health or safety concerns were observed during today's visit. An exit interview was conducted and a copy of this report, Confidential Names list (LIC 811), LIC 809-D, LIC 421FC, and Appeal Rights were reviewed and provided to Administrator Kincherlow.
