Facility Evaluation Report

On 03/17/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct an annual required inspection. LPA was greeted and granted entry by Caregiver Asuncion Fetalvo who was informed of the purpose of the visit. Caregiver Fetalvo reported they informed Licensee Richard Friend of the purpose of LPA's visit. The facility has a fire clearance to care for six (6) non-ambulatory elderly residents of which one (1) may be bedridden. The facility also has an approved hospice waiver for four (4) residents and LPA was informed only one (1) resident is currently receiving hospice services at the facility. During the visit, LPA observed six (6) residents and two (2) staff present. Staff present have a criminal record clearance and are associated with the facility. Licensee Friend's administrator certificate expires on 05/20/2026.

LPA toured the facility with Caregiver Fetalvo. LPA observed the facility is made up of a one-story home with five (5) resident bedrooms, two (2) full bathrooms, a kitchen, dining room, living room and attached garage. Indoor and outdoor passageways are free of obstructions. No bodies of water were observed on the premises. Resident bedrooms had the required bedding, furniture and lighting. Bathrooms had grab bars and nonskid mats in the showers. The hot water temperature in the resident bathrooms measured at 110- and 111-degrees Fahrenheit. LPA observed the facility has more than a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are centrally stored in a kitchen cabinet secured with a magnetic lock. Disinfectants, chemicals, and sharp instruments are also secured in a kitchen cabinet under the sink. Caregiver Ritche Carpio tested one of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA observed fire extinguishers charged and mounted throughout the facility, which were last serviced on 07/25/2025. LPA reviewed the facility's emergency disaster drill log noting their last fire drill was conducted on 01/05/2026.
