The inspector’s account

On 6/19/2025, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by Caregiver, Tyrone Carter who was informed of the purpose of the visit. Licensee, John Young and House Manager (HM), Tyheara Marrow arrived during the visit to meet with LPA. The facility has a fire clearance to serve four (4) ambulatory clients. During today's visit, there was two (2) staff and four (4) clients present.

LPA toured the facility with Marrow and observed the facility is made up of a two-story home with four (4) client bedrooms, two (2) bathrooms, a kitchen, dining room, living room, laundry room, and attached garage. Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had working toilets and wash basins. No bodies of water were observed on the premises. Indoor and outdoor pathways were free of obstruction. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. LPA toured the garage and observed an emergency supply of food and water. Medications are secured in a locked kitchen closet. Caregiver, Tyler Fortune tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard them to be hardwired and operational. LPA also observed two (2) charged fire extinguishers mounted in each floor, which were last serviced on 2/13/2025. The facility's certificate of liability insurance expires on 8/12/2025. Staff present have a criminal record clearance, are associated with the facility, and have a current first aid/CPR certificate. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources (LIC 405) for all four (4) clients while Licensee and HM counted their physical monies and no discrepancies were discovered. LPA reviewed the facility's "Disaster Drill" logs noting the facility's last fire drill was conducted on 5/1/2025 and earthquake drill on 6/6/2025. Complaint procedures, emergency phone numbers, and facility sketch are visibly posted near the dining room. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Licensee Young.
