Facility Evaluation Report
On July 15, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Required Annual Inspection and met with the Administrator, Ricardo Garcia. The facility file review was conducted at the Regional Office, and additional forms were requested and reviewed on site. The CCLD Care Tools were reviewed and discussed at the time of this visit. The facility is licensed for six Elderly Adults and is currently operating within the scope of the license for a 740 facility type. LPA Mixson toured the facility, along with the Administrator, and inspected the facility inside and outside. LPA saw that there were no obstructions or debris to the indoor or outdoor passageways currently. The facility is a single-story home located at 32056 Dayspring way, Temecula, CA 92591. Physical Plant: The facility phone number is (951) 452-1216 and it is operable. LPA reviewed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. Bedroom number one is to be used for caregiver only.
LPA Mixson inspected facility bathrooms, and the hot water temperature tested within regulations. Bathrooms were clean, and appliances were operating appropriately at present. Facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguisher. The fire extinguisher was last serviced January 2026 and was in the green. LPA reviewed the required postings such as "If you See Something, Say Something" the "Personal Rights" and other postings as required. Cleaning supplies and sharp items were locked and inaccessible at the time of this visit.
