Facility Evaluation Report

Licensing Program Analyst (LPA) Seo Jeon arrived to the facility to conduct a case management visit to check on the health, safety, and welfare of clients in care. LPA met with Jesus Gonzalez, IB Investigator, at the facility.

A complaint report was received by the Department on 5-16-2025 regarding Client #1 (C1).

LPA met with Audrey Andrade, Administrator, who allowed LPA entry. There were 49 clients currently at this facility. There were 17 staff on duty during the time of the visit.

LPA toured the facility and did not observe any health and safety concerns. No deficiencies are being cited per California Health & Safety Code and Code of Regulations, Title 22, Division 6.

An exit interview was conducted where a copy of this report was discussed with and provided to Administrator.
