Facility Evaluation Report
On May 15, 2026, Licensing Program Analyst (LPA), Jarred Torres, arrived at the facility unannounced to conduct an annual inspection. LPA met with Case Manager, Dana Garcia, explained the purpose of the visit, and then toured the facility. The facility is a one story building with eleven activity rooms, three offices, two common areas, one isolation room, five bathrooms, and one kitchen. The facility does not have a pool or fire arms. The facility is designated as an adult day program serving adults ages of 18-59 years of age. LPA requested and reviewed documents at the facility in addition to the file review completed at the regional office.
Infection Control: LPA observed the hand washing stations in the facility had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. The facility has a plan on mitigating infectious diseases and training staff on these practices in their files.
Physical Plant: LPA observed the facility 's building , floors, windows, and doors were observed to be clean and fixtures and furniture were present and in good repair. The facility does not have an outdoor activity area. Laundry equipment was observed to be in good working condition. The sharp and dangerous objects were observed to be locked and inaccessible to clients. LPA observed five fire extinguishers with current inspection tags from February 10, 2026.
