The inspector’s account

Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced visit to the facility for a case management visit to follow up on complaint control #18-AS-20251001115652. The LPA was allowed entrance into the facility and met with Dana Garcia, Case Manager. The LPA informed the Case Manager of the purpose of the visit. LPA conducted interviews with the clients in care.

LPA toured the facility and did not find any health and safety concerns for the clients in care. No deficiencies are being cited and no civil penalties per California Health & Safety Code and Code of Regulations, Title 22, Division 6.

An exit interview was conducted where a copy of this report was discussed with and provided to the Case Manager.
