Complaint Investigation Report
On 4/26/25 from 12pm-1pm LPA conducted interviews with witness #1-3 (W1-W3), and on 4/26/25 at 2pm LPA conducted a tour of the facility. On 4/27/25 LPA received copies of crisis prevention intervention (CPI) training for facility staff, and on 4/27/25 LPA conducted a review of C1’s file.
The investigation revealed the following:
Allegation: Facility staff verbally abuse resident.
It is being alleged that client in care is verbally abused by staff daily. On 4/26/25 from 9:00 am- 12pm LPA conducted Interview with L1 regarding the allegation above, L1 denied the allegation above and reported L1 would investigate if a client reported any verbal abuse from staff. On 4/26/25 from 9:00 am- 12 pm LPA conducted Interviews with S1-S3 regarding the allegation above, 3 of 3 staff interviewed denied the allegation above and reported speaking to clients with respect. Per 3 of 3 staff interviewed, licensee would be notified if a client were to report abuse. On 4/26/25 from 9:00 am- 12pm LPA conducted Interview with C1 regarding the allegation above, C1 denied the allegation above and reported being treated well by facility staff. On 4/26/25 LPA unable to conduct interviews with C2-C4 due to communication barrios. On 4/26/25 from 12pm-1pm LPA conducted interviews with W1-W3 regarding the allegation above, 3 of 3 witnessed interviewed denied the allegation above and reported not having any health or safety concerns with the care being provided at the facility. On 4/27/25 LPA conducted a review of C1’s IPP dated: 3/19/24, per IPP C1 has disruptive behavior that interferes with social participation on a weekly, becomes angry and aggressive if C1 does not get C1’s way, physical aggression resulting in injury occurred once within 2024, long history of low frustration level alongside emotional outburst. On 4/27/25 LPA conducted a review of Incident reports dated, 03/04/23, 01/19/24, 10/08/2024, and 4/2/25 per incident reports C1 has assaulted facility staff, school staff, and a police officer.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
It is being alleged that client in care is popped and slapped by staff. On 4/26/25 from 9:00 am- 12pm LPA conducted Interview with L1 regarding the allegation above, L1 denied the allegation above. Per L1, C1 has accused L1 of abused in the past and the allegation was unfounded when investigated by the Inland regional center. On 4/26/25 from 9:00 am- 12pm LPA conducted Interviews with S1-S3 regarding the allegation above, 3 of 3 staff interviewed denied the allegation above and reported staff have not observed any bruising
