Facility Evaluation Report
Licensing Program Analyst (LPA) Beena Singh conducted a case management visit- following a complaint submitted to the department on 10/30/2023 and met with Administrator Vera Adidi to address information obtained during complaint investigation # 56-AS-20231030122857. Department Staff investigated the complaint and allegations were Unsubstantiated, findings were delivered on 03/19/2024. At the time of the visit there were two (2) staff and two (2) clients were present.
Complaint allegations -1. Staff financially abused residents, 2. Staff locked resident in bedroom, 3. facility does not have adequate food supplies. S1 no longer works for the company and was not interviewed.
Regarding allegation the Staff financially abused residents, LPA Singh reviewed Client #1 (C1), in question, financial records and they were found to be in order and accounted for, with no apparent funds missing. LPA Beena Singh reviewed financial records for C#2, C3 and C4 and they were found to be in order and accounted for, with no apparent funds missing. LPA Singh was unable to interview C#1 as he passed away on 10/22/2025.
Regarding allegation that Staff locked resident in bedroom ; LPA Beena Singh toured facility resident bedrooms and observed that the bedrooms do not have a lock on their doors, nor was there no obstructions in front of each door to prevent residents from opening as doors open into the bedrooms. Client#2 was interviewed and stated that no one has ever locked C#1 in bedroom or any other clients.
