Home Care Organization Evaluation Report

Enforcement Analysts (EA), Jane Cong-Huyen with the Home Care Services Branch (HCSB) conducted an onsite inspection for the purpose of a Post licensing visit. The EA met with licensee, Lisa Mendez. The EA observed the posting of the license and operating business hours. Business operating hours are from 10am-3pm, only on Tuesdays and Thursdays.

During the inspection, the EA reviewed personnel records for licensee and Home Care Aides (HCAs) including fingerprint status, registry status, Tuberculosis (TB), and required training(s). Lisa stated she currently only has 6 HCAs actively working for her HCO. EA also reviewed HCO’s business records including document for insurance requirements during today's visit.

EA Cong-Huyen found the HCO in compliance and no deficiencies were cited. An exit interview was conducted, a copy of this report (HCS809) was provided to the licensee/HCO representative, Lisa Mendez, via email.
