Facility Evaluation Report
Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit for a required annual inspection . The LPA was greeted by licensee Blair Alayne Robinson, notified them of the purpose for the visit and was allowed to enter the facility to conduct the inspection.
The physical plant is a single-story structure that consisted of 4 clients bedrooms and 2 bathrooms. The facility has a formal dining room, kitchen, a living room and a garage. Indoor and outdoor passageways were free of obstruction. Dishes and utensils were in sufficient supply and in good repair. Knives and other sharp objects are stored in a locked cabinet in the kitchen. Disinfectant and other cleaning solutions were observed in a locked cabinet in the kitchen area. According to Licensee, there are no firearms on the premises. Clients bedrooms had the required bedding, furniture, and lighting. The dual smoke and carbon monoxide detectors were tested and were observed to be operable. LPA could not check the water temperature because the water heater was turned off and the maintenance person was not available. Licensee agreed to measure the water temperature and send proof to LPA by 08-04-25.
Licensee has never admitted any clients into the facility and has never hired any staff. Therefore, there are no staff/clients files for LPA to review. Licensee/Administrator has an Administrator's certificate with the expiration date of 08-29-26 and a CPR certification with an expiration date of 08-02-2025. LPA observed the following posters: a facility sketch, an emergency disaster plan, a Personal rights, and see something say something. During today's visit, LPA did not observe any immediate violations or concerns.
An exit interview was conducted, and a copy of this report was reviewed and provided to Licensee Blair Alayne Robinson
