Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit to the facility for a health and safety check. LPA met with Caregiver Gloria Reyes, identified himself and discussed the purpose of the visit. A tour of the facility was conducted. No immediate health and safety concerns were observed during the visit.
During today’s visit, LPA interviewed residents and staff and conducted records review.
An exit interview was conducted, and a copy of this report was provided to Caregiver Gloria Reyes
