The inspector’s account

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced annual required visit. LPA met with Administrator, Bashar Myyas, who was informed of the purpose of the visit. At the time of the visit there were (2) staff and (5) residents present.

The facility is a one story home with a detached garage and laundry room. (7) bedrooms and (5) bathrooms. No pools or firearms are housed at the facility. The facility is approved for (6) non ambulatory clients. Physical plant, floors, windows, and doors were observed. At the time of the visit (1) resident room's floor required cleaning. At the end of the visit LPA ensured the floor was cleaned for the resident. LPA also observed in the same resident room linens were being launder, by the end of the visit LPA ensured the bed had clean linens and absorption pads for the resident. Technical notes were documented to ensure resident rooms have clean floors and clean linens. Fixtures and furniture were in good repair were present. The outdoor area was free of hazards. LPA observed hygiene supplies for residents, PPE, clean linens. There are designated locked areas for sharps, dangerous objects, cleaning supplies, and medications. During the time of the visit LPA observed unlocked detached garage where the laundry detergent is being stored. A deficiency was documented and a plan of correction was created. The smoke detector and carbon monoxide are operational, and the hot water temperature 115.5F.
