Facility Evaluation Report

Licensing Program Analysts (LPAs) Janira Arreola and Imaculada Vasquez, conducted an unannounced visit to the facility in order to conduct a case management visit. LPA met with Licensee, Erica Bullock, who was informed of the purpose of the visit. During the visit, LPA conducted interviews, documented observations/conducted a walk through, and conducted records review.

LPAs conducted a walk through of the facility and observed accessible and unlocked cleaning supplies during the time of the visit in a staff room, living room, and hallway. Deficiency was documented and plan of correction was documented. The deficiency was cleared at the time of the visit.

An exit interview was conducted where this report, deficiency page, and appeal rights were reviewed and provided to the Licensee.
