Facility Evaluation Report
Licensing Program Analysts (LPAs) Janira Arreola and Imaculada Vasquez, conducted an unannounced visit to the facility for a case management. LPA met with Licensee, Erica Bullock, who was informed of the purpose of the visit. During the visit, LPA conducted interviews, conducted a walk through, and conducted records review.
During the time of the visit LPAs conducted a review of medication records for the current clients. LPAs were informed the current clients do not have medications lists from their physician's or doctors or on centrally stored lists. The licensee is currently using a medication accounting system that does not have all the required elements in Title 22 section 80075 (k)(7) Health Related Services:
(A) The name of the client for whom prescribed.
(B) The name of the prescribing physician.
(C) The drug name, strength and quantity.
(E) The prescription number and the name of the issuing pharmacy.
(H) Instructions, if any, regarding control and custody of the medication.
Therefore, a citation was issued and a plan of correction was documented with the licensee. An LIC311C Records to be maintained at the Facility for Adult Residential Facilities, was provided the Licensee
An exit interview was conducted with the licensee where this report was reviewed, and appeal rights were provided.
