Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola, Licensing Program Manager (LPM) Carolyn Tuba, and Regional Manager (RM) Reyna Lacey met with Licensee/Administrator Erica Graham Bullock at the Riverside Adult and Senior Care Regional Office for the purpose of discussing ongoing concerns.

The following topics were discussed and copies of the corresponding Title 22 Regulation and Health and Safety Code sections were provided:

Section 80064 Administrator Qualifications and Duties, pertaining to knowledge of and ability to comply with applicable law and regulation.

Section 80044 Inspection Authority of the Licensing Agency.

Licensee agreed to cooperate with Department investigations and inspections, including at unlicensed addresses where the Department may have received allegations of unlicensed care. The Department requested a Resident Roster covering the past (3) years for the facility above. The licensee agreed to submit the rosters by 02/18/2026.

An exit interview was conducted, and a copy of this report was provided to Licensee Erica Graham Bullock.
