Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility in order to conducted a case management on a self-reported incident. LPA met with Case Manager, Cherre Vogel who was informed of the purpose of the visit.

An incident report for 11/12/2025 was received by the Department where (2) were involved in an incident while out in the community. Client interview, staff interviews, and records review of client and staff attendance, client Individualized Program Plans (IPP)'s, Individualized Service Plans (ISP)'s, and LIC602 Physician's Reports were conducted. Record review and interviews revealed incidents of a similar nature were not part of client's known behaviors and staff were present during the incident to supervise clients in accordance to their documented needs.

No immediate health or safety issues were observed or reviewed during the time of the visit. There are no concerns over the reported incident and information gathered at the time of the visit.

An exit interview was conducted and a copy of this report was reviewed and provided.
