Facility Evaluation Report
Licensing Program Analysts (LPAs) Janira Arreola, Imaculada Vasquez, and Jarred Torres conducted an unannounced case management visit. LPA met with Staff, Joshua Schock who was informed of the purpose of the visit. LPAs spoke with the licensee and current administrator, Robert Cantoria during the time of the visit. There was (1) staff and no clients currently in care.
During a visit conducted on 02/19/2026, it was revealed to Department staff that the facility had no clients in care and was using the (4) vacant rooms in the home as short term leases.
During the time of today's visit LPAs spoke with the licensee and staff and was provided with the lease agreements for the tenants and advertisement materials for facility rooms. LPAs were informed that there are currently (3) tenants with short term leases ending by 03/31/2026. Facility also had their facility postings, sharps, and cleaning supplies unlocked for tenant use. Therefore, based on interview observation and record review, LPAs found the facility is operating outside the intended Program Plan to operate as a residential care facility for the elderly and operating short term leases. A deficiency was issued and plan of correction was documented.
Staff #1 (S1) is the property owner and provided a copy of their administrator’s certification and S1 and Staff #2 (S2)'s criminal record clearance during the time of the visit and transfer requests. Therefore, criminal record clearance was verified during the time of the visit.
An exit interview was conducted where this report was reviewed along with deficiency pages and appeal rights.
