Facility Evaluation Report
Licensing Program Analyst (LPA) Toni Nwala, LPA Janira Arreola and LPA Imaculada Vasquez arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by Joshua Schock .LPA began inspection with introduction and visit purpose. There are two (2) staff and no clients.
Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals. There are no pools or firearms in the home. We also conducted record review of staff files. At this time further evaluation is needed and a continuation visit will conducted at a later time.
This LIC 809 report was reviewed with Joshua Schock and a copy was provided at the time of the exit interview.
