Licensing Program Analyst (LPA) Seo Jeon made an unannounced visit to the facility to conduct a Case Management visit regarding a death report received on 08-15-2025. LPA met with Brittany Cabanas, Executive Director, and obtained the pertinent documentation.
LPA conducted a tour of the facility for health and safety. LPA did not find any health and safety concerns at the time of the visit.
There are no deficiencies being cited, per California Health & Safety Code and Code of Regulations, Title 22.
An exit interview was conducted, a copy of this report were provided.
LPA left the facility at 12:30 PM and returned at 01:20 PM.
