Facility Evaluation Report

On 10/8/25 LPA Flores conducted a case management visit at the facility to note deficiencies found during a complaint investigation visit. LPA met with Julie Baez and explained the reason for the visit.

During complaint investigation #18-AS-20250908163554, it was found that on October 15,2022 client #1(C1) was observed with a bruise in the upper left arm and a complaint of pain in the left leg/knee. after an incident with C2. Per C1, C2 had bitten C1. Per interviews conducted with staff C2 may have been having a behavior and C1 resulted with a bite that required medical attention. It is unclear if staff were available to intervene during the incident and/or if there was a plan created previously. However, C1 stated that C2 had previously shown aggressive behaviors towards C1, staff were also aware of previous behaviors.

Deficiencies noted per Title 22 Regulations on LIC 809D.

Exit interview was conducted and a copy of this report was provided.
