Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Palacios made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Director Lynn De Anda who was informed of the purpose of the visit. The facility is comprised of three (3) one story buildings with activity rooms including a music room, offices, conference rooms, isolation room, bathrooms, dining room and medication room. The facility does not have a pool or fire arms. The facility is designated as an adult day program serving adults ages of 18-59 years of age. LPA conducted a tour of the interior and exterior and reviewed facility documents.
LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. The facility has a plan on mitigating infectious diseases and training staff on these practices.
LPA observed the facility physical plant, floors, windows, and doors were observed to be clean and fixtures and furniture were present and in good repair. The facility has outdoor tables and a Garden area where clients can have activities to plant flowers and vegetables. The sharp and dangerous objects were observed to be locked and inaccessible to clients.
LPA observed facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. The facility provides a food program for the clients if is requested but clients are able to bring their own lunch. LPA observed the facilities 2-day and 7-day food supplies which met the requirements. Inspection for smoke and carbon monoxide alarms was conducted on October, 2024 . Inspection documentation was reviewed showing a passing determination. LPA reviewed the facility's Emergency drill noted the facility's last safety drill was conducted on 12/2/24. Facility conducts the safety drills on a monthly basis.
LPA reviewed staff files and training that contained staff criminal clearance and updated training along with CPR/First Aid. Client files were reviewed and possessed all required paperwork.
No deficiencies were cited at the time of the visit. An exit interview was conducted where this report was reviewed and provided.
