Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted am unannounced continuation of required annual visit initiated on 12/12/2023. LPA met with Director, Lynn Vizacarra De Anda who was informed of the purpose of the visit.

Food Service: LPA observed the facilities 2-day and 7-day food supplies which met the requirements.

Physical Plant: Inspection for smoke and carbon monoxide alarms was conducted on 12/12/2023. Inspection documentation was reviewed showing a passing determination.

Record Review and Resident/Staff Files: LPA reviewed staff files and training that contained staff criminal clearance and updated training along with CPR/First Aid. Client files were reviewed and possessed all required paperwork.

No deficiencies were cited at the time of the visit. An exit interview was conducted where this report was reviewed and provided to Director, Lynn Vizacarra De Anda.
