Facility Evaluation Report
This report was created to issue a closure inspection report.
During the required annual inspection, Licensing Program Analyst (LPA) Seo Jeon was advised by the Licensee, Channe Carlos, that they will surrender their license as they no longer intend to operate the facility.
LPA conducted a walk-through of interior and exterior areas of the facility. LPA did not observe any residents or any sign of residents residing in the facility. LPA confirmed with Lapuz and Carlos that there has been no residents since 08-28-2025. Carlos will send formal closure notification to the Department. LPA took possession of the original license from the facility with permission from Lapuz and Carlos. LPA also advised that a new application must be submitted to the Department in case this location is to be operated as a community care facility again.
LPA did not observe any health and safety concerns during the visit.
No deficiencies were cited during the visit. An exit interview was conducted, during which this report was reviewed and provided.
