On 04/29/2025 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit regarding complaint 56-AS-20240122114325. LPA Hernandez met with Staff Florence Nahin and explained the purpose of the visit. LPA Hernandez amended LIC9099 report in order to include all three allegations onto the LIC9099. Previously, only two allegations were added onto LIC9099. Report now includes all three allegations.
