Facility Evaluation Report
On 07/27, 2026, Licensing Program Analyst (LPA), Ahliah Sharp, made an unannounced visit for the purpose of the facility's closure. Care Giver Erman Bularan escorted LPA into the facility and called Administrator (Admin), Lenora Powell for LPA to speak with over the telephone.
On July 22, 2026, LPA received an email from the Administrator and Licensee Tyrone Powell, requesting closure of the facility. On 07/27, 2026, LPA conducted a case management-health and safety check at the facility. They still have all four (4) approved clients in the home, two (2) of which were on grounds at the time of the visit and the other twe (2) were off grounds.
LPA took a tour of the facility; no immediate health and safety concerns were observed. LPA explained to the Admin that they are required to provide the clients and/or responsible parties with a 60-day notice prior to closing the facility indefinitely. Licensee agreed to notify LPA of any changes and/or updates to the closure process.
Licensing will return once facility has confirmed closure.
An exit interview was conducted with Caregiver, and a copy of this report was provided to Caregiver Bularan for Administrator Powell.
