Facility Evaluation Report

Licensing Program Analyst (LPA) Ivashia Wright conducted an unannounced annual required visit. LPA was granted entry and met with Regional Director Ana Gutierrez.

The facility is a one story building with activity rooms, staff offices, bathrooms, and kitchenette. No pools or firearms are being kept at the facility. Smoke and Carbon Monoxide detectors were tested and operable. There are no pools or other bodies of water located at the property.

Infection Control: The LPA observed the hand washing stations in the facility, hygiene supplies, PPE equipment and cleaning supplies to do regular cleaning of the facility.

Physical plant, floors, windows, and doors were observed to be clean and fixtures and furniture were present and in good repair. The facility does not have an outdoor activity area. The sharp and dangerous objects were observed to be locked and inaccessible to clients. The last fire inspection was conducted on 7/15/2026. Water temperature was measured at 112.1F.

Food Service: LPA observed facility kitchenette had the ability to prepare food in a clean environment and possessed equipment in good working condition.
