Facility Evaluation Report
Licensing Program Analyst (LPA) Deborah Mullen conducted an unannounced annual inspection. LPA met with Ana Gutierrez, Program Director and was provided a tour of the facility. Currently the program has clients attending day program on Monday, Wednesday, and Friday for three hours each day. On Tuesday and Thursday clients attend an online zoom class with facility staff. Currently the facility has 8 clients attending the day program.
During the inspection LPA observed the facility to have numerous activity rooms, such as a exercise room, arts and crafts room, computer lab, movie/game room, kitchen area and a library. The facility has a quiet room where a client can wait until they are picked up if they begin to display symptoms of illness. The facility is furnished with chairs, tables and couches for clients comfort. LPA observed each room to be spacious, well equipped with supplies for client activities and to be adequately furnished.
LPA observed the facility to have a set of lockers for clients storage of personal belongings . LPA observed three bathrooms at the facility. The bathrooms appeared safe and sanitary for clients use. The building has hard wired fire alarm system which is inspected annually by the Fire Department.
During the inspection LPA reviewed infection control practices and procedures with the Program Director. LPA observed the facility to be following current infection control guidelines.
The facility appears to be in compliance with Title 22 Regulations and no deficiencies were observed or cited. An exit interview was conducted and a copy of this report was reviewed with and provided to Ana Gutierrez.
