The inspector’s account

On 12/11/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit at the facility. LPA met with staff Carolina Castro and explained to Carolina the purpose of the visit. LPA was granted entry and conducted a tour of the facility alongside Carolina. LPA observed the following during the visit:

Facility is a single-story structure which consists of (5) five resident bedrooms, (1) staff bedroom, (2) two bathrooms, living room, dining room, laundry room, kitchen, garage, and backyard. The facility kitchen was observed to be clean and free of debris. The facility met the required (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. LPA observed an additional refrigerator in the garage that housed overflow foods. The facility maintains a sufficient stock of emergency food and water to meet all resident needs. Knives and other sharp items were maintained in a locked cabinet inaccessible to residents. Centrally stored medication was stored in a locked cabinet in the hallway, inaccessible to residents. Facility provides their own laundry services; all cleaning materials, disinfectants, and detergents were observed to be in the locked laundry room. LPA observed a shaded outdoor area that was fully furnished. Indoor/Outdoor passageways were free from obstruction. Resident bedrooms were equipped with the required bedding, furniture, and functional lighting. Bathrooms were equipped with the required grab bars and slip resistant mats. The facility is maintained at 75 degrees for the client’s comfort. Water temperature measured 109.2 degrees Fahrenheit. There were no bodies of water. Per Carolina, there are no firearms and/or ammunition on the premises.

Resident records review included but not limited to preplacement appraisals, dental, medical assessment, admission agreements, and needs and service plans. Staff records review included but not limited to required training's, health screening, background clearance, personnel reports, and valid First-Aid/CPR Certification.

An exit interview was conducted and a copy of this report was provided to staff, Carolina Castro.
