The inspector’s account

On 03/19/2026, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced case management visit to the facility to assess whether a facility closure was warranted. During the visit, LPA called Licensee Mona Ayad but did not receive a response. LPA was granted entry into the home by the property owner.

LPA toured the home with the property owner and observed the home appeared to be vacant. LPA observed the home to be empty of furniture including beds, drawers, appliances such as the refrigerator, and personal belongings such as clothing in the closets. LPA was informed the utilities were also shut off prior to LPA's visit.

LPA received a return call from Licensee Ayad. Licensee Ayad reported they do not have access to the property located 35876 Neala Lane Murrieta CA 92562. Licensee Ayad reported they are an Inland Regional Center vendor and never received client placement at this address. Licensee Ayad reported they moved out of the home on approximately 02/26/2026 or 02/27/2026. Licensee Ayad reported the Department was not notified nor was a change of location submitted to the Department. As a result, pursuant to Title 22, Division 6, Health and Safety Code 1524(a) the license for this facility has been forfeited. An exit interview was conducted over the phone with Licensee Ayad and a copy of this report was reviewed and will be emailed to Licensee Ayad.
