Facility Evaluation Report
On 3/4/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced visit to the facility for the purpose of delivering an amended version of an original report for complaint control number 18-AS-20250924084053 . LPA met with Staff Florence Ezeh and was granted entry into the facility. LPA explained to staff Florence the purpose of the visit.
During the visit, LPA Flores did not observed any health and safety concerns; no deficiencies were issued nor were civil penalties accessed per California Health & Safety Code and Code of Regulations, Title 22, Division 6.
An exit interview was conducted where a copy of this report was discussed with Staff Florence Ezeh and provided to Administrator
