Facility Evaluation Report
On 1/15/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA Flores was greeted by Administrator Luz Price and explained the purpose of the visit. LPA was granted entry and conducted a tour alongside Luz. During the tour, LPA observed the following:
The facility is a single-story structure which consist of (5) five resident bedrooms, (1) one staff bedroom, (2) two bathrooms, living room, kitchen, dining area, laundry room, garage, and backyard. LPA observed a shaded outside patio with seating. Indoor and outdoor passageways were free from obstruction. LPA observed a television stand which stored crafts and games to encourage resident socialization. Kitchen area was observed to be clean and free of litter. The facility maintained a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable food. Cleaning supplies and other disinfectants were observed to be stored in a locked cabinet under the kitchen sink. LPA observed an additional locked cabinet which stored knives and other sharp items. Medication is centrally stored in a locked closet. Resident bedrooms were observed to have the required bedding furniture and functional lighting. Bathrooms were observed to be in operating condition. Shower areas were equipped with the required grab bars and slip-resistant material. LPA observed a fully charged fire extinguisher near the entrance. There are no pools and/or bodies of water. Per Administrator, there are no firearms and/or ammunition on the premises.
Resident records reviewed included but not limited to signed admission agreements , pre-placement appraisals, needs and service plans, medical assessments, identification and emergency information, and personal rights. Staff records reviewed included but not limited to valid first-aid/CPR certification, criminal record clearance or an exemption, health screenings, TB test results, employee rights, and training verification. During the visit, LPA did not observe any health and safety concerns
An exit interview was conducted and a copy of this report was provided to Administrator.
