Facility Evaluation Report
On 4/8/2024, Licensing Program Analyst (LPA), Janette Romero made an unannounced visit to conduct an annual required inspection at the facility. LPA was greeted and granted entry by Caregiver, Claudia Bonilla who was informed of the purpose of the visit. Licensee, Tasha Hawkins stopped by during the visit. The facility is licensed to care for four (4) ambulatory adults ages 18 through 59. During today's visit, there was two (2) clients and one (1) staff present.
LPA toured the facility's interior and exterior with Caregiver Bonilla. During the tour, LPA observed the facility has more than a 2-day supply of perishable food items and 7-day supply of non-perishable food items, and food is stored in a safe and healthful manner. Medications are stored in a file cabinet secured with a master lock. LPA toured the dining and living/family room area. LPA observed area to be clean and furniture in good condition. Client bedrooms were each furnished with a bed, chair, closet, clothing storage and lighting. LPA observed one (1) client sitting in the living room watching television and the other client sleeping in their room. LPA also observed the facility has board games along with arts and crafts available for client leisure. LPA toured the hallway and observed the hallway to be clean with no pathway obstruction. The facility has carbon monoxide/smoke detectors, which were not tested to not disturb/awake the client sleeping. The facility's last fire and earthquake drill was conducted on 3/2/2024. The facility has backpacks with emergency supplies available for the clients. LPA reviewed random staff and client files. LPA reviewed the Record of Client's/Resident's Safeguarded Cash Resources (LIC405) along with the clients' physical monies and did not discover any discrepancies. Staff files reviewed had the required training records, criminal record clearance, and valid first aid certification. Client files had updated Individual Program Plans and signed admission agreements. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Caregiver Bonilla.
