Facility Evaluation Report
On 01-30-2026, Licensing Program Analysts (LPAs), Abdoulaye Zerbo and Aziz Faizi made an announced visit for the purpose of conducting the final facility closure visit. LPAs was greeted and granted entry by the Administrator, Heddymae Oyson.
The Department was notified of the facility’s intent to close on Jan 20, 2026. The Licensee informed the Department that the closure was voluntary and due to legal issues. During today’s visit, LPAs toured both the interior and exterior of the facility and observed five (5) bedrooms to be all vacant. LPAs confirmed relocation of all the residents.
The Licensee surrendered their original License on 01-30-26 (Effective Date: 02-11-2026). The LPAs explained to licensee that the license will no longer be valid, and therefore no required care and supervision should be provided in the home unless the state approves licensure in the future.
An exit interview was conducted, and a copy of this report was provided to the Administrator Heddymae Oyson.
